Direct Access to Your Physical Therapist
In Wisconsin, there is direct access to physical therapy, which means you can come directly to a PT without having to see your physician first. At SPORT Clinic Physical Therapy we work with you and your physician. If you do not have a physician, and it is indicated that you need one, we will assist in finding you an appropriate one.
When Insurance Requires a Referral
Your insurance may require a doctor's referral before they will pay any benefits. If you have Medicare, you can come and see us directly for an evaluation and to set up a home program. We will provide that information to your primary physician. Medicare requires your MD to agree with and sign/return our plan of care for payment. Your MD may want to or need to see you before active follow up care is started.
No Referral? Free Injury Screening at SPORT Clinic PT
If you do not have a referral and are unsure if physical therapy is appropriate, we offer a free 15-minute injury screen. We will assess your condition, determine your needs and begin a dialogue with your doctor to obtain a referral for physical therapy.
Your Insurance Details
The SPORT Clinic staff is happy to assist you by billing your insurance carrier for services. Please call our office with questions regarding coverage for your specific policy and to verify your insurance. We accept and bill most private insurance and Worker's Compensation plans are also Medicare certified. If you don’t have insurance you will be required to pay for your appointment prior to your session.
We accept cash, checks, VISA, MasterCard, and Discover.
Co-pay fees are due at each visit to SPORT Clinic.
